In the short time since the Portage County Traumatic Brain Injury Clubhouse opened on June 5, 2006, we have served more than 67 Portage County residents in our adult day services program; referred 26 people through phone conversations for appropriate TBI-related services; given 5 presentations to area organizations; distributed concussion information to local coaches and athletic trainers at schools and recreation programs; collaborated with Kent State University programs by hosting 13 field experience and practicum students; hosted 28 monthly Family Support Nights, with professional speakers, attended by over 35 individuals; and hosted 2 Open Houses attended by over 30 area professionals. We have received our Adult Day-Habilitation Certification from Medicaid so that we can serve individuals who are on a structured program, while we continue to be available for drop-in visitors who attend on a less structured schedule, or for specific scheduled activities; and we have received our 501C3 status from the IRS.

It is conservatively estimated that 3,041 residents of Portage County currently live with the consequences of traumatic brain injury (TBI). I am sure you all are aware of the magnitude of the problem of serving adults who have sustained a TBI. Until recently, there have been no services available to meet the needs of adults in Portage County living with TBI.

The Portage County Traumatic Brain Injury Clubhouse is the program that addresses this gap in services. Through our daily adult day services program as well as our educational support group meetings, we are helping meet the educational, social, cognitive, emotional, behavioral, and physical needs of individuals in
Portage County.

We need your assistance so that we can continue providing these opportunities to the Portage County adults living with TBI. Your sponsorship would be greatly appreciated. Please review the attached Sponsorship List, determine the level of sponsorship you feel comfortable with, and then fill out and return the sponsorship form. Your donation is tax deductible.
